Expert Care. Qualified Professionals. Personalized Services.

These are the standards that led Crossroads to Care founder, Patricia Ryan, to develop a premier house call service in the Coachella Valley. Patricia has extensive experience in adult care and understands the unique value of receiving medical services in the privacy of your home.

Patricia holds both a Masters in Nursing and is a certified Adult Nurse Practitioner. Prior to founding Crossroads to Care, she spent years establishing a foundation of care across multiple disciplines. She was a founding member of the Bariatric Program in Kansas City, served as an Emergency Department fast-track provider, provided outpatient care, and established a house call practice in Belfast, Maine. Once relocating to California, she also spent time serving as the Neurology and Stroker Program Coordinator at Desert Regional Medical Center.

Patricia also speaks professionally within the U.S., teaching on a number of health-related topics. In 2013, she received the Educator Award for her work with the American Heart Association.

Using her extensive experience and passion for teaching both healthcare professionals and patients, she instills her quality care standards into every member of the Crossroads to Care family, establishing a foundation of holistic and preventative care standards that are unique to the Nurse Practitioner profession.

Patricia is your patient advocate, coordinating your healthcare needs with your existing health professionals to keep you and your team informed regarding any treatment at home, outpatient care, and within the hospital. The goal is to keep you healthy and at home as long as possible by keeping your medical professionals updated on your entire health picture.

When it comes time to receive care at home, Patricia and the team at Crossroads to Care have extensive resources and services available, from physical therapy, x-ray, and lab draws to 24/7 in-home caregivers.
