Crossroads to Care chronic care patients include those with heart failure, atrial fibrillation, coronary artery disease, stroke, chronic obstructive pulmonary disease, and diabetes. Our goal is to reduce the number of unplanned hospitalizations and emergency department visits. CRTC can monitor and help prevent health events, giving patients the dignity and respect they deserve to stay in their own homes.

Research has shown, by frequently monitoring a chronic illness, there are beneficial effects on patients’ health outcomes. With appropriate care, patients can live longer, stay in their homes longer and have an improved quality of life. Patient mortality and hospitalizations are reduced and frequent in-home care keeps them at home. Additionally, there is a cost savings to the health care system when care is provided in the home instead of in hospitals and other health care settings.

CHRONIC CARE PATIENTS RECEIVE:
  • Same Nurse Practitioner with each visit for continuity of care.
  • Case Management: Assist with insurance billing, work with in home care givers, resource referrals, patient advocacy.
  • Family Involvement: CRTC encourages family involvement and interests in the patients well being.
  • Patient Advocacy: At CRTC we put the needs of our patients first and foremost. We will advocate for the best and most cost effective ways to treat our patients.
  • Billing Insurance First: CRTC participates in Medicare, insurance will be verified prior to appointment.
